Patient Hours:
5:30-10:30 AM M-F
Saturdays 6:30-8:30 AM
Closed Sundays and State and Federal Holidays.
Intakes: M-W-F 5:30 AM
Fees:
Methadone
  • Fees are $85.00 weekly.
  • Patients who pay monthly ($340.00) or bi-weekly ($170.00) faithfully will not be charged for weeks that begin with a fifth Monday.

Suboxone
  • Suboxone Intake fees for Utah are $110.00 and $62.50 per week or $250.00 per month (Does not include Bupernorphine medication).
